Enrollment FAQ’s

LEARNING PATHWAY options for the 2020-2021 school year:

  • TRADITIONAL LEARNING PATHWAY

Students will participate in a traditional school setting with a full-day schedule.Remote learning days will be utilized in the event of inclement weather or exposure/illness resulting in quarantine.

  1. Additional information:
  • All courses/classes taken at school (traditional setting)
  • In-person instruction
  • Traditional academic school day
  • Will utilize Google Classroom for smoother transition to remote learning days (as needed)
  • Will have built in virtual learning days for students and staff to work together on on-line instruction
  • Will transition as needed between in-class and remote learning days
  • BLENDED LEARNING PATHWAY

The blended learning pathway allows for students, grades 7th – 12th, to take part of their classes online and part in a traditional school setting.This pathway would allow for students to participate in extracurricular activities and certain classes on campus (AP, art, band, athletics, etc.) while taking other required or elective courses at home through our on-line program.

  1. Additional information:
  • Requires a semester commitment
  • Combines traditional classes with virtual coursework via Edmentum
  • Student learning plans are directed and monitored by CPS teachers
  • At least one course must be taken on-campus daily
  • Transportation will only be offered during the regular a.m. and p.m. bus routes
  • Transportation throughout the school day will be the responsibility of the parent, guardian, or student
  • Application process required
  • Meals (breakfast & lunch) will be available to all students enrolled in this pathway.
  • Please contact your child’s school if you need assistance with internet access or devices.
  • VIRTUAL LEARNING PATHWAY

The virtual learning pathway is a complete online virtual learning experience for students.

  1. Additional information:
  • Students will report through the CPS Quality Academy
  • Grades PK-6th will use Acellus for online instruction
  • Grades 7th-12th will use Edmentum for online instruction
  • Student learning plans will be directed and monitored by CPS teachers
  • Tutoring, assistance, and assessments could be given at an agreed upon location/time
  • Classes do no require regular on-campus attendance
  • Requires a semester commitment
  • Application process required
  • Meals (breakfast & lunch) will be available to all students enrolled in this pathway.
  • Please contact Quality Academy if you need assistance with internet access or devices.

 

ENROLLMENT OPTIONS FOR 2020-2021

  • ELECTRONIC OPTION- NEW STUDENTS
  • One Page documents: Take a picture with your smartphone.  Pictures of these documents can be uploaded to the CPS Enrollment Submission Form
  • Multi-page documents can be scanned into a single file using your smartphone.  There are many free scanning apps available for download on your smartphone.  One suggested app is called CamScanner. 
  • Once you receive your username and password, log in to WenGAGE  www.wengage.com/Chickasha and click on the Student Records Portal icon.
  • Click Add New Student link and complete Steps 1, 2, & 3, and Submit to Site.
  • Watch your email/phone for communication from Central Enrollment office staff.
  • ELECTRONIC OPTION- RETURNING STUDENTS
  • Log in to your WenGAGE account www.wengage.com/Chickasha and click on the Student Records Portal icon.  Forgot your password?  Click on the link below and complete request form: https://www.chickasha.k12.ok.us/index.php?pageID=278094_2
  • Complete Steps 1, 2, & 3 of the online enrollment and Submit to Site.
  • If you made an address change, please upload proof of residency to verify address.  Options for creating electronic documents from paper using your smartphone:

  • ENROLLMENT CONFIRMATION for RETURNING STUDENTS: Parents of RETURNING students who do not have access to a computer, tablet, or smart phone must attend the ENROLLMENT CONFIRMATION which will be held at the Activity Center, July 14-16, from 7:00 a.m. – 6:00 p.m.  To help with social distancing and guidance from the CDC and health experts, we have designated three (3) days, which will be split up alphabetically by last name.  However, you are welcome to attend any day that best fits your schedule.
  • Tuesday, July 14, 2020: Last name begins with: A-I
  • Wednesday, July 15, 2020: Last name begins with: J-R
  • Thursday, July 16, 2020: Last name begins with- S-Z
  • PACKET DROP-OFF OPTION (NEW OR RETURNING STUDENTS)
  • Collect and make copies of the required documents
  • Complete the fillable CPS Enrollment Packet available on our website at www.chickasha.k12.ok.us and click on the Enrollment tab at the top of the Home page 
  • Print the completed enrollment packet forms and drop them off with copies of the required documents at the east end of the Administration Office, located at 900 W. Choctaw.
  • Summer hours are Monday-Thursday 7:00 a.m. to 5:00 p.m. through July 30, 2020.  Regular hours will resume on August 1, 2020, Monday-Friday 8:00 a.m. to 4:30 p.m.

 

Parents of NEW STUDENTS who do not have access to a computer, tablet, or smart phone can call 405.222.6500 to schedule an appointment with Central Enrollment. 

DOCUMENTS REQUIRED FOR NEW STUDENT ENROLLMENT:

  • Official Birth Certificate
  • Immunization Record
  • Social Security Number/Card
  • Proof of Residency copy (utility bill-electric, gas, water or rental/lease/mortgage agreement)
  • Legal documents, if applicable (custody, guardianship, adoption, name change, etc.)

 

FREQUENTLY ASKED QUESTIONS

1.  Where can I get a copy of my child's birth certificate?

Certified copies of birth certificates can be obtained from the vital records division in the state where the child was born. For a child born in Oklahoma, visit the Vital Records Service Room at the Oklahoma State Department of Health, 1000 NE 10th St., Oklahoma City. The office is open Monday through Friday between 8:30 a.m. and 4 p.m.  For more information or for instructions on obtaining a birth certificate by mail or to order online, call 405-271-4040 or visit  https://www.ok.gov/health/Birth_and_Death_Certificates/Birth_Certificates/

2.  Where can I get my child's immunization record? 

Acceptable immunization documents include a record provided by a licensed physician or a public health authority. These documents must include dates and a signature, or stamp of the physician or health department official.

Exemptions to state immunization law may be granted in accordance with state law and State Department of Health requirements. For more information, please visit http://imm.health.ok.gov

3. What immunizations are required for enrollment?

4.  What can I use as proof of residency?

Acceptable proofs include: Current utility bill (gas, water, electric), rental agreement, warranty deed statement, or mortgage statement.  Telephone, Internet, cable bills, checks and drivers’ licenses will not be accepted as proof of residency. 

Are you living with a family member or friend?  You will need a letter (dated & signed) from family member/friend stating you are temporarily living with them, along with a current utility bill in their name.
